HIGH SCHOOL TRANSCRIPT REQUEST INSTRUCTIONS
Please download the Transcript Request form below and fill it out completely. Print the form, scan it, and email it to adulthighschoolged@mdusd.org, along with a copy of your ID. You can also stop by the high school office to fill out the form in person. If you need assistance, please call us at (925) 685-7340 ext. 67390.
For GED or HiSET please contact them directly for a copy of your transcript.
GED: Go to GED.com, click on the Grads and Transcripts link at the top.
